
What is the Healthways Care Support Program? |
For the benefit of members who live with chronic disease, the CBEBT has partnered with Healthways, Inc., to bring you Care Support, an award-winning disease management program.
The goals of Care Support include
- improving the quality of life for Trust members who live with chronic illness
supporting doctors’ treatment plans
- lowering out-of-pocket costs for Trust members by preventing unnecessary emergency room visits and hospital stays
What is Healthways, Inc.? |
For 25 years, Healthways, Inc., has provided cutting-edge disease and care management programs for hospitals, employers and health care plans like the CBEBT. Healthways programs currently serve more than three million people and their health care providers.
Healthways programs have been recognized for their clinical excellence by the Johns Hopkins Outcomes Verification Program, and accredited by the National Committee for Quality Assurance (NCQA) and the Utilization Review Accreditation Committee (URAC).
How much does the Care Support Program cost? |
Care Support is provided to Trust members and their eligible dependents at no additional cost.
How does the Care Support Program work? |
1. Automatic enrollment
Healthways uses state-of-the-art technology to review CBEBT benefit claim data and identify members with chronic conditions, such as diabetes, asthma and pulmonary disease. If you’ve received medical services for an ongoing illness or condition, you will be automatically enrolled in the Care Support Program.
If you are contacted by Healthways but do not wish to participate
in Care Support, you must opt out by calling Healthways toll free at 1.888.870.2433. Healthways may contact you regarding more than one health condition.
2. Your Care Support team
Once enrolled in Care Support, each Trust member is paired with a Care Support team of experienced nurses, dieticians and other specialized health care professionals. These highly trained clinicians help you
- understand your condition(s)
- review medications and treatments
- set goals and develop action plans
- understand your doctor’s recommendations
Your Care Support team is available seven days a week, 24 hours a day to answer any questions you may have.
3. The role of your primary health care provider
Healthways Care Support does not replace your doctor’s treatment plan. Rather, your Care Support team works with you and your health care providers to make sure you receive the highest quality care possible.
4. What to expect
As a participant in the Care Support Program, you can expect the following:
- a welcome letter and a phone call from your Care Support team
- access to the online health assessment tool, CareSteps, which allows you to create a customized report on the state of your health
- periodic calls from your Care Support team to support your self-care efforts
- reminders to schedule important appointments
- communication between your Care Support team and your doctor regarding standards of care and treatment plans
- an invitation for your doctor to participate when you interact with Care Support clinicians by phone
- newsletters and educational materials
What if I have a chronic illness but don’t wish to participate in Care Support? |
The Care Support Program is voluntary. However, if you are being treated for a chronic or ongoing illness, you will be automatically enrolled in the program. If you have been contacted by a Healthways representative but decide you would rather not participate in Care Support, you must opt out of the program. To do so, please call Healthways toll free at 1.888.870.2433.
If you choose to opt out, you are opting out for the current identified condition only. You may be contacted in the future for other conditions as they are identified. We encourage you to take part in Care Support, however, your decision to opt out of the program will not affect your other CBEBT health care benefits.
