On-Site Screening Information

 

You are taking the first steps necessary in securing a healthy tomorrow by participating in Wellness Inc.’s Individual Screening program your employer is hosting at your screening site.

  Each Wellness Screening takes approximately 15-20 minutes where you will fill out the Health Power Profile™ questionnaire, get your blood pressure taken, and then have your blood drawn for the Panel C diagnostic test. You will also have the option of taking the additional PSA and ColoCare® tests.
 

The PSA (Prostate Specific Antigen) is free for CBEBT male members 50 years of age and older, or those with a history of prostate cancer. All others not qualified pay just $39.50. The ColoCare® is a take home, self-test to detect hidden blood in the stool. This test can aid in the early detection of gastrointestinal disorders and colon cancer. This test is recommended for men and women age 50 and over and is available for $5.00.

Remember to fast for 8-12 hours before the Panel C blood test. Two or more glasses of water is acceptable, but no other food or drink is allowed.

Each screening features the Panel C diagnostic tests, a series of 35 blood tests that test for the early warning signs of various diseases, disorders and risks, as well as blood pressure and body composition analysis, and a health risk assessment. The results of the screenings are 100% confidential. Upon completion of the screenings, you will receive your personal Health Power Profile™ Report and Detailed Test Results. And new for 2008 is the Physician Summary Screening Report. This valuable condensed screening report summary has been specially designed to be shared with your primary care physician and placed in your patient record.

You will also have access to the test results via a secure website, MyWellnessInfo.com. This personal health portal is a confidential Web site that has more than 5,000 searchable topics to allow participants to investigate causes, symptoms and treatments for many health related issues. The portal allows you 12 months of secure online access, past and present wellness screening results, highly credible health resources, assessments and tools, a drug reference guide and much more!

 
New for 2008, members who participate in the Wellness screening will also receive the Wellness Health Coach benefit and the Healthier at Home® self-care book with instructional DVD.
 

The Wellness Health Coach will help guide you to develop a personalized plan of action to help you achieve you health goals. Your Coach will provide you with hand-selected educational materials at a pace that’s just right for you. All coaching sessions with your Wellness Health Coach may occur via email, telephone, and/or a variety of online activities. When you receive your Wellness Screening results packet, you will find information on how to enroll in the Health Coaching program.

 

When you participate in the Wellness Screening, you will receive a copy of the Healthier at Home® self-care book with instructional DVD. This easy to use resource contains helpful advice and information on some of the most common health problems. This guide is designed to help you reduce your health care costs and improve the quality of your life. The Healthier at Home® resource provides information on how you and your family can stay healthy, tools to communicate more effectively with your health care providers and guidelines for deciding when and how to care for your illnesses or when to seek professional care.

The Wellness Screening is considered to be a preventive health test and is therefore 100% covered by the Trust for enrolled participants and their dependents age 18 and older. The screening is in addition to the preventive benefits already provided by your plan. Employees will be asked to show proof of Trust Membership at the time of the screening by simply presenting their EBT medical ID card. For those not enrolled in the Trust, the screening is still available at a discounted rate of $100. You will need to provide payment at the time of the screening. Accepted forms of payment include cash, major credit cards, and personal checks made payable to Wellness Inc.
