At-Home COVID Testing Resource Information
As previously communicated, Christian Brothers Services is working to streamline the process of a new mandate requiring private insurance plans to cover the costs of at-home COVID-19 test kits.
As indicative of COVID-19, the situation remains fluid and we will provide information as it becomes available.
Testing Coverage and Reimbursement FAQs
UPDATE 1/28/2022 How do I get a test? There are three ways to get your test.
- Go to COVIDtests.gov. You can order four, free at-home COVID-19 test kits through the United States Postal Service. All you need to do is register and provide residential address. No payment information needed. Shipment is expected within 7-12 days from the date ordered.
- Go to your local in-network pharmacy or order for home delivery through the Express Scripts Pharmacy and use your pharmacy benefit for $0 cost.
- No need to file a claim, using your pharmacy card is the easiest way to receive a free test through Express Scripts (ESI).
- Be sure to visit the pharmacy counter, not the general check out, and show your ID card.
- Login to your account for a list of participating pharmacies. Click on "my prescription benefits" then "locate a pharmacy."
- Purchase at retailer or online vendor and file for later reimbursement. If you weren't able to purchase your at-home COVID-19 test at the pharmacy counter, or happened to be charged, you must complete the following to submit a manual claim:
- Retain dated receipt
- Provide UPC code from test kit box
- Submit a special COVID-19 test reimbursement form found here. If you have already submitted a form, it will be processed accordingly.
- If your receipt does not have the UPC or NDC printed on it, you MUST put the UPC and/or NDC on the form.
- You will receive reimbursement of up to $12 per test, regardless of purchase price, beginning Jan. 28. If you made a purchase between Jan. 15 and Jan. 27, you will be reimbursed for the entire cost shown on receipt.
- Please do not send materials directly to Christian Brothers Services. Reimbursement will be handled through our pharmacy vendor ESI. ALL CLAIM FORMS MUST BE SENT TO ESI at the following address: Express Scripts, ATTN: Commercial Claims, P.O. Box 14711, Lexington, KY 40512-4711
In addition, you will be able to order at-home COVID-19 tests through Express Scripts Pharmacy for home delivery by logging into your ESI account where you will find further instructions. Please note that in the short term there will be limited supplies and tests may take up to four weeks to arrive.
Please note due to the complexity of the situation, processing time for these claims may be longer than normal.
UPDATE 1/18/2022 Class 2 Device Recall FLOWFLEX SARSCOV2 Antigen Rapid Test.
The FDA recently placed an urgent recall for the FLOWFLEX COVID-19 Antigen Rapid Test, with the following lot numbers:
UDI #: 6921756492427
COV1080232 - Expiration date: 05/13/2023;
COV1095004 - Expiration date: 09/03/2023.
Please note this recall is only these specified lot numbers.

UPDATE 1/15/2022 Every home in the U.S. can soon order 4 free at-home COVID-19 tests. The tests will be completely free—there are no shipping costs and you don't need to enter a credit card number.
Ordering begins January 19.

UPDATE 1/14/2022 At-Home COVID-19 Tests to be Covered Beginning January 15 This past Monday, January 10, 2022, the Department of Health and Human Services, along with the Department of Labor, released new guidelines pertaining to at-home COVID-19 test kits. These new guidelines mandate that private insurance plans begin covering the costs for these at- home COVID-19 test kits, beginning, Saturday, January 15, 2022.
In compliance with this new policy, the Christian Brothers Trusts (CBEBT and CBRMT) will cover up to eight, over-the-counter COVID-19 diagnostic tests authorized, cleared, or approved by the U.S. Food and Drug Administration (FDA), per covered individual, per month, purchased on or after January 15.
Only the following tests are FDA-approved and authorized for reimbursement:
- Inteliswab COVID-19 Rapid Test
- Binaxnow COVID-19 AG Self Test
- Quickvue At-Home COVID-19 Test
- Ellume COVID-19 Home Test
- Flowflex COVID-19 AG Home Test
No other brands will be covered at 100% under the plan at this time. You may purchase these at-home, over-the-counter COVID-19 test kits through any available pharmacy or retailer.
Christian Brothers Services is working swiftly to streamline the process for reimbursement for such tests and will notify members and participants as soon as the method is determined. Please retain all proofs of purchase and refrain from sending those to our office until we have finalized a formal process. We will send further communication regarding the process as it becomes available.
We are diligently working with Express Scripts Inc. (ESI), the Trusts' pharmacy manager, to add at-home COVID-19 tests to its covered drug list, making it simple for participants to obtain a free test kit using their pharmacy benefit in the near future.
Within the next few weeks, you will be able to obtain these approved kits through our network of pharmacies by showing your insurance ID card, with no upfront cost and no claims forms. As the situation remains fluid, please be patient as we work to make this happen.
